Step 1: Register
If you have not already done so, register online to receive the latest updates from iQ Academy.
Step 2: Apply
Once registered online, complete and submit all required forms by fax, mail, or email. iQ Academy Washington students are required to submit forms and documentation prior to acceptance at our school.
The Enrollment Application includes the following forms which may be submitted separately:
- Enrollment Cover Sheet
- 2 page Student Registration form
- Certificate of Immunization or Statement of Exemption
- Statement of Understanding form
- Enrollment Agreement 2009-2010
- Request for Sending Personally Identifiable Records
- Course Selection Form
- FOR ALL STUDENT WHO DO NOT LIVE WITHIN THE EVERGREN PUBLIC SCHOOLS BOUNDARIES:
- Full time students must submit an Application for Student Transfer to Nonresident District form, to be signed by both parent/guardian AND THE STUDENT'S RESIDENT SCHOOL DISTRICT (the school within whose boundaries your physical address falls)
- Part time students must submit an Interdistrict Agreement form, to be signed by both parent/guardian AND THE STUDENT'S RESIDENT SCHOOL DISTRICT (the school within whose boundaries your physical address falls)
Other Required Documents for new students
- Transcript - Each High School student will be required to fax, mail, or email a copy of transcript from previous school attended, including WASL scores if the student is entering from a Washington school. Our academic counselor will need this information to help students with required course placement.
- WASL (or other standardized testing) Scores - Each student must fax, mail or email in a copy of their WASL scores, if this information is not included on the transcript
- Special Education (504, IEP & Psychological Report/Evaluation) - Students with a 504 or IEP (Individualized Education Plan) should provide a copy of their IEP or 504 plan. Our Special Services Department will need to review these documents.
- Guardianship Papers (temporary or permanent) - Students who have a temporary or permanent legal guardian will be required to provide documentation that states the guardian is able to assist in medical and educational needs. When custody arrangements exclude a parent (sole custody), we will need a copy of the court order/divorce decree stating custody arrangements.
Step 3: Course selection, Orientation, and Welcome Kit
Once your Enrollment Application is processed, your academic advisor will contact you to help you select your courses.
- Once your Enrollment Application is approved and processed, your school counselor will contact you (or you may contact us) to help you select your courses and to approve your schedule.
- An invitation to register for an Orientation will be emailed to the email addresses provided on your application. This information will also be sent though the mail with your Welcome Kit.
- A Welcome Kit will be mailed to your home with information about how to log in and get started with your courses, and about how to check out an iQ Academy issued laptop (if applicable).
